In your situation, the person on the phone was just apologizing for the background noise, not specifically for the coughing. The conversation can go something like this, Bony (over the phone): "I'm sorry for the noise. My wife has this awful cough." Chris: "Not a problem. I hope she gets better!" When somebody in the room coughs, you don't have to say anything. But if you want to help, you can ask, "Are you ok?" "Do you want some water?" "Do you want a cough drop?" When somebody sneezes, you can say, "Bless you."
