美国纪行3：9.13-9.19：工作
9月13日 Sunday
today is Sunday,the weather is still very nice.
After dinner. eliz told me that she still need to work this afternoon. so I decided to go to downtown by myself by bus. you know I have bought the bus pass for 12days, but I still have no chance to use it .
I should save the money, so I took something to eat.
Before going out, I asked Eliz to comfirm the bus no.31 can took me there directly. then I found the timetable on the net, photo in the DC. I waited the bus for 20minutes. the bus drivers are mostly black, but they kind and patient too.I asked him to give me a signal when I should get off. the bus took me there with passing by the miller’s brew factory and the harley Davidson motorcycles company, but only passby, next time I hope I can have a look in them and make some different experience there. the miller’s looks like in a historical place in Europe.
I walked into the park in a emergency entrance, but in the end I founded that I should buy a ticket to attend the Indian-american festival. it is really very shocked. so many Indian people here, they all came from all over the US. some dance competition，most of the Indian children can dance in a traditional way to match the music, they all dressed in Indian clothing..

今天是周六了，可我还是在补作业。本来想逼自己用英文思考和写作，看来还是要有感觉，有时就是没有，也不能强求，告诉老公我的想法，他说不要逼自己，写多少算多少，学得到多少算多少。前几天同他聊天学了一句话，vice versa。So his English is also very smart!
I found a cap made with the rabbit skin for the cap body and raccoon for the tail, I like it, but the size is too small to me. the saler is an old man from Texas, he give a name card and told me if I really want to buy it , I can write the email to him.
When I waited in the bus stop, I saw an old man riding a harley motor to pass by with a “young lady”in his backseat.after a few minutes, they came back and asked me “would you like to take a picture for me?” so I took a fun picture,a luxury motor with a old couple,bue the lady in hot pants.
On the way home,I found some tree became colorful, Eliz told me the first one which changes color in autumn is the sugar maple tree, some leafs turn to orange some yellow. but the maple tree in front of her house is ugly,only one color.
I went to sleep early because I felt a little bit headache.
9月14日Monday
today Dr.W pick me up in the morning with his diamond benz again.
the first pt today is a GDM take medication, her first son is spina bifida. now he is 10 yrs. he had surgery theray, but still sit in wheelchair. but he really seems happy. this defect has 2-5% recurrence rate, especially in GDM.
The second is a >35yrs,18wks, the first thing is the genealogy, they all make a detail for every consultation. they tell the patient, you can choice what you like ,the serum screening and the ultrasound are all the screening, if you want to know yes or no, you should take the AMA test for the karyotype. we also find a small hole in her baby’s heart, like a ventricle septum defection ,told her to make an appointment with the feto-cardiologist.
Desiree told me she took part in a party in someone she met from the ‘meetup.com’. the condominium—an apartment belong to certain person, who buy it, was built up on the Milwaukee river. everything is luxury. Desiree said everyone brought a champagne or wine to there, they just talked and talked and have fun. most of them never know each other before. she also told me ,there are so many group on this website, I also can try to match some friend there.
The 3rd pt has 23months twins. one is a down’s baby boy, another is a normal clever girl. this time her risk for trisomy21 is 1:4700 but Tri18 is 1:3, she had already taken the AMA test ,the karyotype is normal. I discussed with Dr.W,uE3,aFP,HCG,PAPP-a are all low level, but the screening result is positive. it is unbelievable. Why it happened, After all we find the reason is her gestation age estimated by US 2 weeks earlier than her due date.
Today we had 2 more twins.

周二和周三早上起得很早，这两天有conference. One day is only for OB, another day is for OB&GYN. But the form just like a discussion. the residence prepare the pt’s history, and the Director gave some questions to the residency.and all the partners and some register nurses attended the meeting in the morning.
I only can catch a little meaning about what they said.the first is a DM the discuss about the complication of the baby, the first is the CHD’s VSD,it should be checked in 20+-weeks by US.another is the shoulder dystocia. 2nd case is about Chron’s desease with pregnance. Deep Venous Thrombosis in 28weeks…
Today I work with Cindy, she answer a case result to the ‘thefetus’. the answer is fetal teratoma in left ventricle. before she write the email, she check it with her notebook and the internet. after a couple of hours, she got the reply. Excellent! good job! this really make her so exciting, she told the good news to everybody, because the chief of the net is very authoritative in echo-cardia.Dr.Jeanty.
On 17th sept, cindy also give an answer to the week case, interruption of the aorta arch distal to the left septum., incredible that this time Dr.Jeanty replied her in person. ask her where is she from?
Cindy also share another case with me-Heterotaxy.
In 2pm, I watched a FLOC operated by Dr.De Lia. He set up TTTS institute in Wisconsin. The pt is transferred from IOWA in 29 weeks. he make a cut in up belly 5cm, and make a troka on the uterus 3mm to input a fiber lens with laser .the view is not very clear. I really do not know according as which rule, Dr D to choose which placenta vein to do the laser surgery. but I am so luck that I have a new chance to see the surgery again in next Monday. I should read some paper to understand what he do,why he do in this way.
It is very interesting that some of the doctors in PNAC have not watched the surgery before. this is also their first time as me.
Sept 18th
1. ring chromosome and 2 nature miscarriage,she use the LOVENOX for her hypertension.
2. go back home at noon and have a nap.
3. I have canceled my plan to yellow stone national park this time.
Sept 19th Sunday
I drived a auto bar car to Mayfair Mall with Dr.Poremski. He told me that I should make a completely stop in front of the ‘stop’signal in the corner of the cross. And he order a bootblack to clean his shoes for tonight’s gathering with childhood’s friends in Chicago, told me that they had made a plan to visit china in next summer. I hope they can arrange the schedule.
I used the laundry in Eliz basement for the first time, everything is auto here.
